RE: Important Notice: Permanent Closure of Gill Primary Care
Dear Patient,
We are writing to inform you that after many dedicated years of serving the Stockton and Lodi communities, Gill Primary Care will permanently close its practice, effective September 21, 2026. It has been an absolute privilege and honor to care for you and your family. We are deeply grateful for the trust you have placed in our clinical team over the years.
Your Ongoing Healthcare Needs
Your health and continuity of care remain our top priorities during this transition. Because continuous medical care is vital to your well-being, we strongly encourage you to select a new primary care physician as soon as possible.
To locate a new provider:
Contact your health insurance provider for a list of in-network primary care physicians in the Lodi/San Joaquin County area.
Or
Contact the San Joaquin County Medical Society or the California Medical Association for lists of practicing primary care doctors accepting new patients.
Medical Records & Prescription Refills
Your medical records remain completely confidential and secure. To ensure your new physician has seamless access to your health history:
Requesting/Transferring Records: You may request a copy of your medical records or authorize us to transfer them directly to your new healthcare provider. Please complete and sign the Medical Records Release Authorization Form digitally at the following web address:

Your form will be submitted automatically once signed, and no further action is needed from you. Record requests submitted on or before November 21, 2026, are free of charge. Requests made after this date will incur a processing fee.
Your Right to Revoke: You may revoke your authorization for release of records at any time by notifying us in writing, except to the extent we have already acted on it.
Prescription Refills: Please review your medication needs now. Our office will continue to fulfill necessary prescription refills through September 21, 2026. After this date, prescription refills must be handled by your new primary care provider.
